Military Affairs Council (MAC) 2023 Community Service Awards Nomination Form

The Cocoa Beach Regional Chamber of Commerce's Military Affairs Council (MAC) annually sponsors the Community Service Awards (CSA) Program to recognize the volunteer efforts of active-duty military personnel in our civilian communities. It is a pleasure and privilege for us to honor those military members who have given so much of themselves to our local community.

3 award winners will be recognized at the Chamber's annual "Wine for Warriors" event hosted in December each year. Each award winner and their guest will be provided with complimentary attendance and will receive a Community Service Award and a gift card courtesy of the MAC.
